Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     As a MDS Coordinator you will be responsible for coordinating MDS (Minimum Data Set) as required per State & Federal guidelines. You will also provide residents with direct care to assure quality of life in a home like environment while offering additional support to family and visitors. You will maintain a professional behavior while functioning as an interdisciplinary team member.

    Qualifications:

    • Graduate of accredited school of nursing (LPN) with current state license.'
    • Must be LPN and have experience in MDS 3.0 and skilled nursing.
    • One (1) or more years clinical experience SNF preferred
    • Prefer 2 or more years in long term care facility or assisted living
    • Knowledge of Medicare, RUGS's state and federal regulations.
