Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     The RN MDS Coordinator conducts and coordinates development and completion of the resident assessment in accordance with the requirements of the state of Oregon, and the policies and goals of this facility.

    Required Qualifications:

    • Education sufficient for licensure

    • Oregon Registered Nurse License upon hire.

    • 1 year - Must have, as a minimum, 1 year of experience as a MDS Coordinator or case manager in a hospital, long-term care facility, or other related health care facility and 1 year experience with the RAI process in long term care facility.

    • Must have experience in rehabilitative and restorative nursing practices.
