Minimum Data Set RN at Providence Child Center in Portland, OR - This position will work, full-time, day shift

The RN MDS Coordinator conducts and coordinates development and completion of the resident assessment in accordance with the requirements of the state of Oregon, and the policies and goals of this facility.

Required Qualifications:

• Education sufficient for licensure

• Oregon Registered Nurse License upon hire.

• 1 year - Must have, as a minimum, 1 year of experience as a MDS Coordinator or case manager in a hospital, long-term care facility, or other related health care facility and 1 year experience with the RAI process in long term care facility.

• Must have experience in rehabilitative and restorative nursing practices.
