Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Overview:
We are seeking a highly organized and detail-oriented individual to join our team as an MDS Coordinator. As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely medical documentation and utilization management. This position requires strong knowledge of physiology, case management, and acute care standards.
Duties:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents
- Collaborate with healthcare professionals, including doctors, to gather necessary information for accurate documentation
- Conduct utilization review to ensure appropriate levels of care are provided to residents
- Review documentation for accuracy and completeness, making necessary corrections or updates as needed
- Maintain compliance with regulatory requirements and standards related to medical documentation
- Collaborate with interdisciplinary teams to develop and implement care plans based on resident needs
- Provide education and training to staff regarding proper documentation practices
- Participate in quality improvement initiatives related to medical documentation and utilization management
Qualifications:
- Bachelor's degree in nursing or a related field (preferred)
- Valid nursing license (RN or LPN)
- Proven experience in utilization management, case management, or similar roles
- Strong knowledge of physiology and acute care standards
- Familiarity with eClinicalWorks or similar electronic medical record systems
- Excellent attention to detail and ability to maintain accuracy in medical documentation
- Strong organizational skills and ability to prioritize tasks effectively
