Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. As the Minimum Data Set (MDS) Coordinator, you will be responsible for assuring the timely completion of
accurate assessments and interdisciplinary care plans that meet State guidelines for all Maine Avita
Communities. Included is identifying resident acuity and needs, helping to determine specific care needs and
communicating needs / expectations to responsible parties.
