Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. The successful MDS Coordinator will coordinate and assist with completion and submission of accurate, timely, and full assessments of residents. He/she will submit assessments according to government/insurance standards, review Final Validation Reports to ensure all assessments are accepted, and coordinate with the Business Office to guarantee accurate and timely billing.
The MDS Coordinator will work with the lead MDS Coordinator, the facility DON/ADONs, and other staff to monitor residents for significant changes in condition. MDS staff will also assist with reviews of Comprehensive Care Plans and changes to those plans, and provide education to direct care staff on those changes.
MDS Coordinators must have the following licensure:
Curent Indiana Licensed Practical Nurse or Registered Nurse license
