Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. RN/LPN LEAD MDS Coordinator: Full Time
Responsibilities Include:
The MDS Coordinator will be responsible for updating and maintaining care plans according to state and federal regulations. This person will also need to work with the interdisciplinary team to provide education to the nursing team related to MDS changes or any issues with MDS accuracy. Meets with the DON and NHA to review and keep abreast of any changes, issues etc.
Qualifications:
MDS Coordinator experience
SNF/LTC experience
Licensed Nurse in MD
