Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Purpose of Position
The primary purpose of this position is to manage the Resident Assessment Instrument (RAI) process of Forest Hills of DC. An additional and equally important purpose of this position is to assure the need of resident is clearly defined and outlined throughout the documentation in the MDS process.
