Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Job Description:
As the Minimum Data Set Coordinator (MDS Coordinator), you will conduct and coordinate the development, completion and transmission of the resident assessment in accordance with the requirements of this state and policies and goals of this facility.
Qualifications:
- 2-4 years’ of MDS experience
- Nursing degree from accredited college
- Current and unencumbered LVN/LPN or RN license in the state
- Knowledge of clinical standards of practice, regulations and reimbursement governing long term care and Medicare participation
- Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
