Mental Health Clinician - MSW provides clinical mental health counseling and case management services to patients. Completes patient assessments to identify social and emotional challenges.   • The Mental Health Association of Monmouth County seeks a full-time Clinician for its Community Support Services (CSS) program. The Clinician reports to the Director of Community Support Services. 


    Responsibilities:

    • Complete clinical assessments, formulate treatment plans, and conduct 90-day reviews for all consumers.
    • Contribute to the development, implementation, monitoring, and updating of individualized rehabilitation plans in accordance with N.J.A.C. 10:37B-2.4.
    • Manage overall case management for assigned consumers.
    • Coordinate and oversee service provision.
    • Assist the CSS Director with administrative duties as needed.
    • Provide therapeutic rehabilitative skill development per NJAC 10:37B-4.2.
    • Offer services tailored to meet consumers' needs, in alignment with NJAC 10:37B-4.6.
    • Perform crisis intervention as outlined in NJAC 10:37B-4.4(a)24.
    • Ensure timely completion of all required documentation in consumer files, adhering to policies and applicable regulations.
    • Establish familiarity with and involvement in the lives of assigned consumers, including contact information, roles, and contributions.
    • Assist consumers in utilizing available community resources to address their needs.
    • Enhance consumer independence by facilitating skill development, either through direct modeling or community referrals for instruction.
    • Attend Peer Supervision meetings and agency staff meetings.
    • Visit consumers in state psychiatric hospitals as referred and participate in treatment team meetings when requested.
    • Perform other duties as assigned.


    Skills:

    • Assess general clinical status and interpret consumer charts when available.
    • Identify clinical crises that may require emergency intervention.
    • Engage effectively with consumers and contribute to the development of their living skills.
    • Network with a variety of formal and informal community resources and supports.
    • Educate consumers on mental illness and other functional impairments.
    • Address substance use/dependence issues and educate consumers on these matters.
    • Demonstrate safe driving habits as specified by NJMVC.
    • Maintain awareness of available clinical resources and the means to obtain them.
    • Collaborate with consumers to create crisis plans utilizing community 24-hour services and Partners in Independence staff.
    • Educate consumers on following crisis plans developed with their peer advocate.
    • Exhibit excellence in verbal and written communication skills.


    Education:

    • Master’s degree in mental health, health, or other relevant human services disciplines, with an LSW or LAC required.
    • Two years of related work experience.
