Summary Job Description:
Licensed clinician with experience in mental health, family supports and care coordination. Individual therapy and group therapy responsibilities with a person-centered approach. Team and systems-based support for Members.
Support the linkage development with community ‘care partners’ and coordinate Member engagement in these partnerships. Cocreation of Care Plans and Aftercare planning with members which align with their Life Worth Living Goals is an essential job function and responsibility for this role.
