We are looking to hire an MSW for Boca Raton 

Responsibilities

• Provides direct counseling to patients to ensure their wellness
• Serves as an effective liaison and communicator between patient and families
• Assists families with Medicaid applications as needed
• Provides a comprehensive evaluation of the psychosocial status of the patient's condition/environment and develops a social work care plan
• Ensures that required documentation is complete and is in compliance with regulations and standards

Qualifications

• Currently licensed in the State of Florida as an MSW Social Worker
• MSW degree from an accredited Masters of Social Work program
• Current CPR Card from the American Heart Association
• At least one year of experience in Home Health
• Advanced knowledge in mental illness, personality development, and knowledge in the types of psychiatric treatment/therapy
• Familiar with community resources information
