Job Duties/Requirements:
The Cedar Home Health and Hospice Medical Social Worker will provide supportive and medical social services to patients and their families in accordance with an appropriate plan of care. The Medical Social Worker will assume responsibility for the development, implementation, and management of psycho-social services provided to home health and hospice patients and families in the patient’s home.
- Master’s degree from an accredited school of social work.
- Prior Home Health/Hospice experience.
- One (1) year of social work experience in a healthcare setting.
