Social Worker - MSW Qualifications:

• Licensed in the state of Minnesota as an LGSW or LISW (Master's-Level Social Worker)
• At least one year of clinical Social Work experience
• Experience with Electronic Medical Records (EMR) preferred

Social Worker - MSW Responsibilities:

• Conduct Social Work home visits to assess patients' psychosocial status
• Provide Social Work services in the home, including short-term individual counseling, community resource planning, and crisis intervention
• Collaborate closely with all members of the health care team in developing the plan of treatment

Job Type: Part-time

Pay: Up to $52.00 per hour
