Required: MSW from an accredited graduate school of social work.

Current licensure by the State of Connecticut as a Licensed Master Social Worker.

Licensed Master Social Worker (LMSW) must achieve (LCSW licensure) within two years of date of hire.

Required: Competence in meeting the needs of patients admitted to the hospital for treatment of complex medical issues, chronic disease and/or rehabilitation regardless of age or disability as determined through the interview process, previous work experience, and education.

Required: One year clinical experience in a medical setting.

JOB SUMMARY
-
The social worker is a member of an interdisciplinary treatment team responsible for meeting the psychosocial needs of the patient. Responsibilities may include patient/family assessment, counseling, education, case management, advocacy and planning services. The social worker may also facilitate the development of a discharge plan and may coordinate after care continuity. The social worker provides education to staff, patients and families as necessary.

PHYSICAL DEMANDS

The job requires carrying of documents, books, and materials; occasionally the transport of patients, frequent walking, standing, sitting during many counseling/therapy sessions or meetings usually no more than ninety minutes; reaching and bending to access file drawers or cabinets, possible participation in physical restraint of patients, lifting and carrying of children. Manual dexterity is required to write type or use the computer and to access the dictation system.

Job requires primarily working indoors with an occasional need to be outdoors. Job requires working around many other people and the ability to interact with a variety of disciplines on a continuous basis. Schedule fluctuation is frequent and is often unpredictable due to census, documentation requirements and/or program development. There is exposure to body fluids, contagious disease and occasionally to blood-borne pathogens and radiation.

- Demonstrates a working knowledge of current theory/practice; integrates such information into practice
- Completes all documentation according to discipline/unit standards
- Participates in the development/implementation of an interdisciplinary plan of care
- Provides supportive counseling to patients/families according to treatment plan
- Assists in the discharge process in collaboration with the treatment team, patient, and family
- Develop/maintain an understanding of the culture, traditions, values, beliefs that relate to health care and to decision making
