Job Title: Social Worker - BSW - Housing Case Manager

Social Service Professionals, "the professional's choice" for careers in Social Work and Therapy is looking for multiple Social Workers (BSW) to work as a Housing Case Manager in Marin County near San Rafael, CA. Choose between positions in the Wrap Around Services (conducting mental health assessments and connecting clients to supportive services) or Rapid Rehousing (addressing immediate housing needs). In these positions you'll assist chronically homeless individuals with a history of co-occurring disorders. Both positions involve a combination of on-site and field based work. Responsibilities:

-Evaluate patients’ psycho-social needs and the impact of illness on those needs;

-Evaluate patients’ family/social situation to support a treatment plan;

-Utilize community resources to assist patients’ psycho-social;

-Document history, care plans and prepare reports as needed.

Experience Requirements:

-1 to 2 years of experience as a Social Worker (BSW).

Education Requirements:

-Graduation from an accredited university or college with a Bachelor's Degree in Social Work, or related field.

Qualifications:

-Current AHA-approved CPR card

Skills:

-Skills in critical thinking, problem-solving, logic and reasoning, judgment, time management techniques and data entry;

-Ability to make improvements or take corrective action;

-Observe, receive, and otherwise obtain information from all relevant sources;

-Organize, plan, and prioritize work;

-Communicate with people outside the organization, represent the organization to patients, clients and the public;

-Deal calmly and effectively in high stress situations;

-Develop constructive and cooperative working relationships with others, and maintain them over time;

-Work within a team as well as independently;

-Speak, read, and write effectively as appropriate for the needs of the audience while demonstrating the proper use of grammar.
