POSITION SUMMARY:
This position will promptly and efficiently retrieve process, distribute, maintain, collect, and store patient medical records; sort and forward to appropriate recipients, both internal and external; organize and evaluate medical records for completeness and accuracy. Continuation of this position is contingent upon funding allocations.
DUTIES AND RESPONSIBILITIES::
- Retrieve and forward patient medical records information to internal staff and physicians.
- Organize and analyze medical records for completeness and accuracy.
- Assemble all medical records according to the chart order set by the guidelines.
- Obtain, create, and maintain patient charts, histories, and related records and files; this includes manual and electronic medical record system.
- Check for misfiles and out-guides which; indicate that a medical record has not been returned and trace missing files.
- Ensure that all clinical administrative procedures, records and documentation are in compliance with relevant regulations, accrediting agencies and behavioral health records’ standards DHS 75 & DHS 35 standards; compile and prepare operational and activity reports, in compliance with procedural requirements.
- Compile and prepare operational and audit reports to ensure providers are remaining in compliance with regulations.
- Pull charts for next day or emergent appointments.
- Follow the written facility policy and procedure for releasing of medical information to other organizations and/or to patients.
- Ensure all information released is to be complete, accurate, and done in a timely manner.
- Sort and file all correspondence pertinent medical information. Determine what needs to be filed or scanned.
- Retrieve dictation records and convert/import documents to appropriate electronic medical record and/or distribute to appropriate provider.
- Scan appropriate documents into the computer to assist in creating the patient’s Electronic Medical Record.
- Ensure medical records vault is locked at the end of the day.
- Contribute to a team effort.
- Adhere to all Tribal Personnel Policies and Procedures, Tribal Standard Operating Procedures, and Area and Program Strategic Plans and Policies.
- Maintain strict department security, confidentiality, and quality to meet professional standards of the department.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT::
- Frequently stand; handle or feel; reach with hands and arms; talk, hear; and walk in tight aisles.
- Occasionally sit, stoop, kneel, crouch, or crawl; lift and/or move up to twenty-five (25) pounds.
- Work is generally performed in an office setting.
- Not a smoke free environment.
- Evening and/or weekend work; extended hours and irregular shifts may be required.
- A Tuberculosis (TB) Screening and/or a 2 step TB Skin Test is required within thirty (30) days of employment.
STANDARD QUALIFICATIONS::
- Knowledge of applicable Oneida, federal, state, county and local laws, regulations, and requirements.
- Knowledge of medical terminology.
- Knowledge of modern office practices, procedures, and equipment.
- Knowledge of effective practices for maintaining records management.
- Knowledge of regulations and laws governing the maintenance of official records.
- Skill in operating computers, faxing, scanning, and copy machines.
- Skill in using and maintain Microsoft Office programs specifically Excel, Word and Outlook.
- Skill in preparing and maintaining patient records for accuracy.
- Skill in providing strong customer service.
- Ability to communicate effectively both verbally and in writing.
- Ability to establish and maintain professional relationships with the public and co-workers.
- Ability to handle multiple tasks and meet deadlines.
- Ability to prepare and maintain reports for accuracy.
- Ability to understand and follow oral and written instruction.
- Ability to work independently.
- Complete Oneida Certification on Reporting Child Abuse and Neglect training within ninety (90) days of employment.
- Complete and maintain BLS Training within 3 months.
- Work remotely as needed according to the Telecommuting SOP.
- Must adhere to strict confidentiality in all matters.
- Must complete Health Insurance Portability and Accountability Act (HIPAA) training within thirty (30) days of employment and annually thereafter.
- Must be willing and able to obtain additional education and training. :
- Must pass a pre-employment drug screening. Must adhere to the Nation’s Drug and Alcohol-Free Workplace Policy during employment.
- Must comply with Caregiver Law and Grant conditions criminal history checks as a condition of employment to include fingerprinting.
- Employees are required to have proof of immunity or dates of two (2) doses of MMR and proof of immunity or dates of two (2) doses of Varicella prior to starting in any position within the Oneida Comprehensive Health Division. Any refusal of vaccination(s) or failure to provide proof of immunity may disqualify the applicant.
- Must pass a background security check with the Oneida Nation in order to meet the Employment Eligibility Requirements, Tribal/State Compact and/or Oneida Nation Gaming Ordinance as they pertain to the position. A temporary license or Gaming License issued by the Oneida Gaming Commission is required as a condition of employment and continuing employment within the Oneida Nation Gaming.
MINIMUM QUALIFICATIONS::

- High School Diploma, HSED Diploma or GED Certification; applicants age fifty (50) and older are exempt from this requirement;
- One (1) year experience in a position responsible for maintaining files or record keeping or as a Medical Records Technician.
