JOB DESCRIPTION:
The duties include, but are not limited to:
1. Gathers patient information by collecting demographic information from a variety of sources; interacting with registration areas and physicians' offices.
2. Initiates the medical record by creating and processing the patient care record folder.
3. Maintains record availability by processing charts into the department; using chart mark-off procedures; facilitating chart location activities.
4. Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes.
5. Delivers charts to assigned areas of the practice by following established routing procedures.
6. Keeps health care providers informed by communicating availability or unavailability of the record.
7. Maintains quality results by following practice standards.
8. Maintain thorough, accurate, and professional quality of work.
9. Maintain strict confidential for both internal (employee) and external (non-employee) patients in accordance with Valley EyeCare Center policies and HIPAA, accessing only information needed to appropriately handle the request.
10. Maintain professional boundaries with all patients, visitors, and other staff at all times.
11. Interacts harmoniously and effectively with others, focusing upon the attainment of organizational goals through a commitment to teamwork.
