    The Medical Records Clerk will perform all tasks related to the initiation and completion of medical records to include filling, retrieving, and distributing medical records. 


    ESSENTIAL JOB FUNCTIONS: 

    1. Manages medical records by sorting, filing, and collecting a variety of medical records and information such as admission slips, laboratory and pathology reports, operative notes, clinic notes, discharge summaries in the patient medical records in the appropriate sequence and following established procedures. 
    2. Closely examines patient medical records for completeness ensuring all required information is included. Follows up to obtain missing records. 
    3. Manages incoming faxes and timely categorizes, distributes, and attaches documents to appropriate parties following established priorities and procedures. 
    4. Manages outgoing faxes for successful transmission. 
    5. Maintain daily census lists and record discharge dates and deaths appropriately; check census information against existing records for accuracy and completeness verifying information and making corrections as necessary. 
    6. Answers telephone and takes requests for medical records or information; sort requests according to predetermined priorities and provides routine information of a general nature such as history numbers, admission or discharge dates and medical record locations to authorized requestors. 
    7. Assist in researching patient billing discrepancies and assist risk management. 
    8. Demonstrates thorough knowledge of medical terminology and pre-authorization practices. 
    9. Processes all Release of Information requests, working with finance and risk management when required. 
    10. Performs other duties as assigned by supervisor.

    Requirements

    1. High School diploma or GED is required.
    2. 1 years’ experience working in a family practice or hospital medical records setting required. 
    3. 1-3 years of medical records of computer software required. 
    4. Graduate of certified medical office training course preferred.
    5. Certified Nursing Assistant, Medical Assistant preferred.
