    FULL-JOB DESCRIPTION

    Position Definition

    To manage and facilitate a variety of agency case reviews which promote quality services, adherence to state and federal rules and promotes best practice standards for services to children and families. The Case Review Coordinator participates in the monitoring of agency practice and outcomes.

    Reports To

    The Case Review Coordinator reports to the Deputy Director.

    Employment Status

    This is a full-time position carrying a 40-hour per week work regularly scheduled within the hours of 8:00am to 5:00pm Monday thru Friday, except legal holidays, with flexibility of early morning or evening hours as needed to complete job responsibilities. Some weekend work may be required. The supervisor may expect the worker to use an Alternative Work Site.

    Principal Responsibilities

    1) Perform Case Management Services

    • Facilitate and monitor agency quality review systems.
    • Facilitate and monitor agency case review system for children in custody.
    • Facilitate and monitor agency case review system for children prior to removal or at risk of removal.
    • Facilitate and monitor agency Semi-Annual Reviews/90 Day case review for in-home and custody cases.
    • Facilitate, monitor, and document Family Team Meeting and Permanency Roundtable meetings based on best practice models.
    • Make home visits with families and other participants before the FTM to prepare for the meeting.
    • Provide on-call agency service for after-hours coverage as determined by the first call beeper rotation.
    • Schedule meetings/case reviews in the family home or in the community when appropriate to meet the needs of the family.
    • Manage groups with objectivity and respect and ensure meetings are completed timely.
    • Stay abreast of current best practice and lead initiatives within the agency.

    2) Perform Assessments

    • Monitor and provide data relating to practice and rule compliance issues.
    • Identify specific practice or rule compliance issues which need to be addressed by the agency. Example CPOE, CAPMIS, SACWIS.
    • Review data applying to rules and practice pieces and measure outcomes.
    • Coach staff on application and measurement of practice pieces, forms, and documentation.
    • Coach staff on leading child and family meetings.
    • Review agency case records to measure for quality and compliance.

    3) Present Information

    • Maintain written summaries of various reviews and meetings.
    • Document the planning that comes out of the meeting into a document format.
    • Provide copies of the plan to the necessary parties.
    • Responsible for ensuring proper documentation and managing the information in the case records.
    • Provide reports/training to staff, regionally and or at state meetings about practice issues, and or work on state committee.
    • Testify in court.

    4) Collaborate and Coordinate with Multiple Stakeholders

    • Manage systems of planning, notification, and documentation of various agency case reviews.
    • Responsible for the scheduling of various agency reviews including Semi-Annual Reviews (SAR’s), 90 Day Case Reviews, Case Reviews, Family Team Meetings, Permanency Roundtables, Matching Meetings.
    • Responsible to ensure, or work with the assigned clerical staff to ensure, notice to all appropriate parties, and documentation of those efforts.
    • Follow the necessary steps for paper flow with reviews filed in court, and copies forwarded to the necessary parties.
    • Participate in agency initiatives as assigned.
    • Participate in coverage for the team.
    • Attend team meetings.
    • Attend specific job-related training.
    • Attend state committee meetings as assigned as a participant.

    5) Utilize Appropriate Supervision

    • Meet with supervisor regularly (bi-weekly) as scheduled to discuss work management, case direction, issues, and questions.
    • Share records, letters, case information with supervisor.
    • Share information with supervisor on independent decision making. Develop agenda for supervisory conferences.
    • Consult with supervisor on a regular basis regarding scheduled meetings.
    • Share all records and casework activity with supervisor.
    • Discuss case dispositions and resolutions prior to entering data systems.
    • Show balance between over-dependence on supervisor and independence in decision making.
    • Utilize supervision during crisis situations.
    • Prepare an individualized training plan along with supervisor and show ability to evaluate own practice.
    • Participate in and contribute to agency meetings, discussions, and conferences.
    • Discuss information entered data systems and monitored for quality and compliance.
    • Attend staff meetings and required in-service training.
    • Consult with supervisor and assistant prosecuting attorney.
    • Review recruitment ideas and plans with supervisor on a regular basis.
    • Keep supervisor informed of critical issues that may arise between scheduled conferences.

    Other Duties and Responsibilities

    • Ability to break down information in incremental steps through discussion and demonstration and share this with the family.

    Required Qualifications

    • BA in Social Work or a related degree
    • Valid driver’s license and Ohio Revised Code (ORC) compliant auto insurance coverage
    • Candidate must pass a comprehensive background assessment.
    • 3 (Three) years of Ongoing court involved casework.

    Preferred Qualifications

    • Licensed Social Worker preferred.

    Required Training

    The Case Review Coordinator is to attend all required training within the first year of employment including required Core Training. An additional 12 hours of domestic violence training is required in the first two years. ACCS staff are to attend safety training, as offered through the RTC, and any other agency scheduled training. The Case Review Coordinator are expected to upgrade knowledge and skills through advanced training as recommended by their supervisors and as reflected on the individual ITNA.

    Caseworkers are expected to use computer programs including Microsoft Word, SACWIS, Outlook, and any other computer programming that will be utilized by the agency in the future. Orientation to these programs will be provided by the agency.
