Litigation Support Manager oversees the team that uses technology to support an organization's litigation efforts. Requirements for the Care Manager:
· Bachelor’s Degree in Human Services or a related field or a CASAC along with 2 years of experience in the Human Services field.
· Valid NYS Driver’s License required.
· Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required.
Responsibilities for the Care Manager include:
· Provide Health Home Services on site and in the field to a caseload of individuals with the goal of reducing the rate of preventable hospitalizations through linkage, connection and engagement with all necessary community-based resources and services for behavioral and physical health care.
· Coordinate the development of the Individualized Care/Service Plans/Plan of Care.
· Meet the Health Home and DOH requirements for outreach and engagement & active care management.
· Ensure and coordinate the completion of necessary assessments to identify client physical & behavioral health needs.
