Licensed Practical Nurse - Long-Term Care jobs in Illinois

Licensed Practical Nurse - Long-Term Care administers nursing care in a long-term care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Long-Term Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Long-Term Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Long-Term Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     Provides prescribed medical treatment and personal care services to patients in the clinical setting by performing the following duties supporting the nursing philosophy. May include day, evening, night, weekend, holiday, and/or on-call hours.

    Responsibilities

    Essential Duties and Responsibilities

     

    • Collects patient data and documents in the medical record.
    • Assists with procedures, examinations and testing as needed.
    • Reviews physician's orders, lab requests, or follow-up needs with patient.
    • Documents services provided in the medical record.
    • Reads and understands complex medical documentation.
    • Provides patient treatments as ordered.
    • Observes patients and reports adverse reactions to medication or treatment
    • Administers specified medications as allowed by policy and procedure, licensure, and State Practice Act.
    • Participates in patient teaching
    • Provides basic healthcare instruction to family and patient.
    • Performs venipuncture to collect blood specimens.
    • Participates in QA & I.
    • Performs other duties as assigned.

    Education and Experience

     

     

    Graduate of an accredited LPN Program.Experience in clinical setting preferred

    Certification and Licensure

     

     

    Licensed as a Licensed Practical Nurse in state of practiceApproved CPR or BLS certification required within 90 days
