    Description

    LICENSED PRACTICAL NURSE

    The Licensed Practical/Vocational Nurse (LVN/LPN) provides nursing care students/patients and teaching to students/patients, families, and school staff. He/She also is supervised by the Registered Nurse.

    Qualifications

    - Graduate of an approved program of practical/vocational Nursing

    - Licensed as a licensed graduate Practical Nurse in the District of Columbia

    - Two (2) years of experience as a nurse in a school, pediatric acute care, public/community health, clinic setting preferred

    - CPR Certification (American Heart Association)

     

    Chronic Care Management

    • Provide education to all students having chronic illness to further patient understanding of condition and treatment choices, with an emphasis on patients with diabetes, asthma, allergies, seizures, ect.
    • Perform student medication administration and treatments as assigned per physician's medication treatment authorization plan of care.
    • Participates in the planning and coordination of students care in conjunction with the RN, school administrator of designee, and SST team to support 504 plans, IHP development, and student support team plans.
    • Develops and follows the individual health plan for students with chronic conditions each student assigned to received care.
    • Meet with other multi-disciplinary school team members on a monthly basis to review high risk students and goals that are impacted by student health, as requested by school team.

    Patient Care

    • Triage students who present to the health suite/clinic
    • Provide direct services as outlined in student medication/treatment authorization form, IHP, 504 plan.
    • Provides nursing care and conduct patient education, including:
      • Dietary and lifestyle modifications related to DM, asthma, allergies, seizures and other chronic conditions
      • Medication administration and teaching (primarily for DM, asthma, allergies, seizure)
      • Asthma care and education
      • Preventative health screenings (e.g. vaccines, vision, hearing, blood pressure checks, COVID-19 screenings etc.)
    • Administration of vaccines.
    • Manage patient medication refills and renewal of medication assistance.
    • Participate in community outreach, vaccine clinics, and education programs as requested.

    Responsibilities

    - Participates in the planning and coordination of students care in conjunction with the RN, school administrator of designee, and SST team to support 504 plans, IHP development, and student support team plans.

    - Follows the individual health plan for each student assigned to received care.

    - Observes the students; evaluates the care of the patients; applies his/her knowledge of nursing skills to the students to whom he/she is assigned

    - Accurately reports and records the patients condition and care, including signs and symptoms which may be indicative of change

    - Prepares clinical/progress notes on the day of the student visit and incorporates it in the clinical/documentation record daily; prepares student care report

    and provides to schools administer.

    - Assists the students with the activities of daily living while teaching appropriate self-care techniques

    - Provides and maintains a safe environment for the students

    - Provides optimum physical and emotional environment

    - Assists the Registered Nurse in complex nursing situations and performing specialized procedures

    - Participates in case conference, in-service, and other programs and meetings as needed

    - Performs within limits of scope of practice, preparation and experience

    - Communicates timely with parents and school administrators a student illnesses, injuries, and changes in condition

    - Perform student medication administration and treatments as assigned per physician's medication treatment authorization plan of care. An LVN/LPN may perform only those treatments for which they are qualified by training and experience to perform.

    - Maintains regular attendance; performs all duties in a timely manner; Attends all meetings as scheduled and/or directed by supervisor or CEO.

    - Follows all procedures and adhere to policies as outlined in the Employee Handbook.

    - Access Social Services and Education resources as appropriate to ensure safe, efficient integration of student services and support.

    - Respects confidentiality at all times.

    Organizational Relationship

    - Reports to RN Administrator and/or delegating RN

    - Reports to designated school administrator while in school setting

    Functional Abilities

    - Able to communicate verbally and in writing to the extent required by the position

    - Able to physically perform the duties required by the position

    Physical Demands

    • Able to move about inside and outside the health suite to provide care to students, access file cabinets, medications, etc
    • Able to ascend/descend stairs to respond to provide student care and respond to individuals requiring emergence care (e.g. CPR or emergency care/assessment).
    • Able to stoop, kneel when responding to an individual requiring emergency care (e.g. CPR, seizure care, ect).
    • Able to observe small text (e.g. read insulin syringe and observe medication for administration)
    • Able to lift up to 25lbs
    • Must be able to exchange accurate information with students, staff, and families regarding student related information
