Job Details Overview:

As an LPN with us, you'll be the ray of sunshine in our patients' lives, providing direct care and support under the guidance of our registered nurses.

Responsibilities:

Your responsibilities will include:

• Providing direct patient care as outlined in the State Nurse Practice Act.
• Implementing care plans initiated by our clinicians.
• Documenting patient care accurately and promptly.
• Educating patients and their families/caregivers on diagnoses and care plans.
• Collaborating with the team to coordinate home health services.
• Using equipment and supplies effectively and efficiently.
• Embracing opportunities for personal and professional growth.
• Assisting with other duties as assigned by our Registered Nurses.

Qualifications:

We're looking for LPNs who shine bright with the following qualifications:

• Graduate of an accredited practical nurse or vocational nursing program.
• Preferably, at least one year of nursing experience.
• Currently licensed as an LPN or LVN in the State.
• Demonstrates excellent verbal and written communication and organizational skills.
• Must be a licensed driver with a reliable and insured automobile.

Functional Abilities:

• Ready for lots of walking, standing, and occasional lifting.
• Able to bend, stoop, and move with ease.
• Good visual acuity and hearing.
• Can read 12-point or larger type.
