Licensed Practical Nurse - Home Care jobs in Oregon
Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. What you will do:
- Plan and deliver nursing care to residents in accordance with current company, federal, state, and local standards, guidelines and regulations
- Ensure assigned residents receive their medication and treatments as ordered in the respective care plans
- Act as a leader to support a team of CNA's
- Completion of all assigned assessments as ordered by RCM and DON
Must have:
- Current and unencumbered license to practice as an LPN in the state (NEW GRADS WELCOME!)
- Responsible for Med carts
- Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
- Ability to speak, understand, read, and write in English proficiently

As required by state and local law, Meadow Park Care provides a reasonable pay scale to include the hourly or salary range that Meadow Park Care expects to pay for roles they may be hired in Oregon, as stated below:
