Licensed Practical Nurse - Home Care jobs in Connecticut
Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Responsibilities may include the following but are not limited to:
- Traveling to patient's home and providing "Best Practice" nursing care
- Checking vital signs
- Documenting in a complete and confidential manner
- Evaluate the patient’s wellbeing, reporting any needs to the Director of Clinical Nursing or emergency services
- Providing personal care and meals
- More advanced nursing such as chest tubes, IVs, colostomy, urinary catheter management are also available
- Giving emotional support to client and families in times of crisis
Requirements:
- Valid LPN license
- Annual physical within one year
- PPD or Quantiferon within one year or chest x-ray within five years
- COVID vaccine card with booster
