Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. - Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
- Providing interventions with a focus on achieving realistic client outcomes.
- Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
- Administering medications as needed.
- Maintaining client records showing systematic assessments, planning interventions, and evaluations.
- Initiating referrals as needed to other health team members.
What we’re looking for:- Current license as a Licensed Practical Nurse (LPN).
- Current CPR certification.
- Access to reliable transportation.
- Maintains confidentiality in all aspects of the job.
