JOB SUMMARY: The Licensed Clinical Social Worker acts as part of the patient care team and provides emotional and mental health counseling to groups or individuals, builds strong rapport with clients and makes accurate assessments of their problems whether mental/emotional or physical. Collaborates with care team, treating clinicians, counselors, and nurses to develop, plan and manage treatment. Refers patient, family, or client to other agencies for treatment to help in recovery from physical or mental illness. Monitors, evaluates, and records client progress with that of treatment plan. Keeps detailed and concise records of each patient during the course of their treatment.
REQUIREMENTS: LCSW, Master's Degree and at least 2 years of supervised clinical social work.
