    Description

    Position Summary

    The behavioral health clinician promotes efficient coordination of patient care through collaborative practice within the current health care team’s facilities. Provide social work case management functions including brief social work interventions and behavioral health and addiction counseling for patients and families that address personal, environmental, and socioeconomic issues in order to maximize emotional, social, and physical well-being and the appropriate use of healthcare and community services across the continuum. Responsible to work with providers, nurses, and all other team members to recognize and address bio-psychosocial issues that inhibit positive health outcomes as well as patient and family strengths that may contribute to wellness and recovery.

    General Responsibilities

    • Provides professional expertise and consultation to the provider and team
    • Performs assessment and brief treatment for a wide range of behavioral health and addiction issues using brief interventions
    • Coordinates and works with other team members to ensure the efficient and effective flow of patients through the clinics.
    • Ability to maintain established productivity standards
    • Evaluations and diagnosis of children, adults and elderly patients for mental health and addiction needs.
    • Provides situational counseling or basic behavioral plans for an array of medical needs, such as dieting, medical compliance, and smoking cessation.
    • Utilizes appropriate billing codes for behavioral health and substance use interventions.
    • Focuses on functional outcomes, uses flexible treatment strategies, and provides patients with functional interventions they can practice on their own.
    • Writes behavioral plans as necessary for patients to self-manage.
    • Demonstrates a general knowledge of health medicine and has a functional knowledge of psychotropic medications.
    • Develops and documents notes based on assessment information and documents appropriately including identified goals measureable objectives, and time frames for achievement..
    • Ensures the position functions cohesively with primary team members.
    • Receives patients from the provider willingly, and makes follow-up consultation with the provider upon completion.
    • Answers referral questions and provides the provider with adequate evaluation, diagnosis, and treatment recommendations.
    • Operates comfortably within the primary care culture and does not force traditional mental health modalities into the primary care setting.
    • Knowledgeable with Evidence-Based brief interventions such as: Problem Solving Therapy (PST), Cognitive Behavioral Therapy (CBT), Interpersonal Counseling (IPC), Behavioral Activation (BA) or similar intervention.
    • Reports previously unknown medical needs to medical team members.
    • Readily provides unscheduled services when asked.
    • Educates other staff on the community resources as well as seeks out new community resources for patient needs.
    • Facilitates patient referrals to substance abuse treatment programs and specialty mental health treatment services in a manner that increases the likelihood that those patients make contact with those programs. Other duties as assigned.
    • Work with other collaborative programs such as Michigan Collaborative Care Implementation Support Team (MCCIST).

    Qualifications


    Education and/or Experience

    Required:

    • Master’s Degree in Social Work from a CSWE accredited program
    • Masters Social Worker License (LMSW) with valid State of Michigan license
    • -or-
    • Master’s Degree in Counseling from an accredited program
    • Licensed Professional Counselor (LPC) with a valid State of Michigan License
    • Michigan Certification Board of Addiction Professionals (MCBAP) required or willingness to obtain.
