Summary:  Responsible for the overall mission of assigned Family Coordinator grant.  Contributes significantly to program development, quality assurance, service coordination and support services for youth referred to the program.  Works collaboratively in carrying out the goals and functions of the program.  Provides programmatic and consultative assistance to other programs.

Essential Duties and Responsibilities include the following.  Other duties may be assigned to meet business needs.

• Develops strategy to complete objectives identified within state designated Statement of Work
• Educates agency programs and community resources about family programming.
• Is responsible for general oversight of program functionality
• Provides community programming and outreach across Westbrook’s eight county region
• Facilitate the formation and the continuation of regional family advisory committee
• Partner with other child serving agencies to increase cross-referral and utilization of existing resources within the state
• Engage and support families in change processes meant to increase well being
• Promote and support positive family engagement
• Completes required grant reporting and other selected data sharing processes
• Attends all meetings and trainings identified within the statement of work
• Complies with federal, state and company policies, procedures and regulations
• Maintains confidentiality of all information and complies with HIPAA regulations.
• Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior
