Commonwealth Land Trust Medical Case Manager
Location: Windsor House, Lawrence
SUMMARY: Commonwealth Land Trust provides permanent supportive housing and social services to individuals who have experienced chronic homelessness, substance addiction, and physical and mental health conditions. The Medical Case Manager provides case management services, addictions relapse prevention, mental health and HIV/AIDS health concerns support, crisis response, and educational groups to clients. Case managers provide services on-site where the clients reside. Bi-lingual Spanish preferred.
COMPENSATION FOR THIS POSITION: $55,000 to $65,000 per year, plus generous benefits.
