To be a Laboratory Services Director typically requires 3+ years of managerial experience.   • Position Summary: The Director of Clinical Services is a licensed clinician (Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Licensed Master-Level Social Worker (LMSW), or Speech and Language Pathologist (SLP)) who has graduated from an accredited institution and is licensed to practice in the state of Michigan. She/he is responsible for supervising and managing home health services provided to homebound patients in accordance with physician orders and plans of care. The Director of Clinical Services is a clinical leader who oversees all clinical staff and manages patients under the Oakland Home Care and Oakland Therapy branches. The Director of Clinical Services will have a passion for providing the highest quality of care in order to achieve positive patient outcomes.

    Main Tasks & Responsibilities:

    • Oversee the clinical aspects of the agency ensuring the high standards of our accrediting bodies, Medicare, Medicaid as well as any state or federal guidelines are being me to ensure our patients receive the highest quality outcomes
    • In partnership with the Executive Director, develop and institute agency’s clinical objectives and goals
    • Based on physician orders, patient, and family needs, coordinates interdisciplinary healthcare services to deliver great patient outcomes, manage and track patient outcomes
    • Serve on utilization review committee, oversee ongoing performance improvement program
    • Develop and maintain policies/procedures in accordance with federal & state regulations
    • Ensure quality assurance and applicable regulatory compliance by monitoring completion and quality of visit notes, coordination of services, and plans of care
    • Perform chart audits regularly, determine and correct issues within audits
    • Supervise and manage care given to patients in accordance with physician orders and plans of care
    • Assist with recruiting, hiring, evaluating, and terminating personnel; assist with orientation & training as needed
    • Conduct monthly patient case conferences, in-services, staff meetings, and maintain all appropriate documentation
    • Observe the utmost confidentiality and safeguard all patient related information, including Protected Healthcare Information (PHI)
    • Work with the Executive Director in identifying budgetary requirements and determining appropriate use of resources
    • Investigate any problem relating to patient care or conditions which might harm the patient and/or well-being

The Director of Clinical Services is not a treating role, however all staff may be asked to jump in when necessary to help make sure our patients are taken care of. The Director of Clinical Services role is a leader to the clinical staff, office staff, and also out in the community. It is important that she/he work to build and maintain relationships within the communities we serve.     Job Requirements:

    • Maintains a state clinical license in good standing with the appropriate/accredited degree based on the clinical discipline (RN, PT, OT, MSW, or SLP)
    • Three to five years of experience in home care, preferred
    • Two to three years of experience in a health care supervisory position, preferred
    • Proficiency with computer applications such as electronic medical records (Home Care Home Base preferred) and Microsoft Office products
    • CPR Certification, required
    • Knowledge and understanding of the home care Conditions of Participation
    • Knowledge and ability to apply community health principles and practices
    • Ability to supervise, guide, and develop skills and performance of personnel
    • Ability to enlist cooperation of many people in furthering a program
    • Ability to deal effectively with high levels of stress

    Physical Requirements:

    • Constant use of manual dexterity, auditory, and visual skills and gross motor skills with frequent use of bi-manual dexterity and fine motor skills
    • Ability to follow written and oral instructions and procedures
    • Ability to reach, bend, twist, squat, kneel, push, and pull
    • Ability to lift/carry up to 50 pounds. 80 pounds of pressure may be placed on the associate through transfers using transfer belts or mechanical assists
    • Ability to perform repetitive movements including simple grasping, pushing, pulling, and fine manipulation
    • Frequent sitting, standing, and walking
