  • Intake Coordinator position description: Under the direct supervision of the Administrator/ Director of CTC Services, and in collaboration with the CTC Clinical Team, this position is the key to managing the daily census and determining the schedule for admissions, referrals and discharges. Working with the Case Managers, Clinicians and Nurses, this position will assist in determining through a medical screen, if the patient is appropriate for admission to the CTC level of care or if a referral to the continuum of care offered by Well Care or another entity is more appropriate for this person.

    This role acts as a representative of the CTC and the organization when interacting with the public, community partners, referral sources and the potential patient and family. It is therefore important to use professionalism and excellent customer service, during all interactions, regardless of with whom.

    Qualifications: This position of Intake Coordinator requires advanced knowledge of assessment and evaluation of medical and behavioral health issues. The person must understand how to deal with intoxicated people, law enforcement, Nursing staff and the rest of the team. Although not Case Managers, they will need to assist in the referral of those evaluated for services at the CTC, yet are not appropriate and need to be referred to other programs in coordination with the Nurse, Case Manager, or others at the CTC or the Clinic. The ability to multi-task, manage processes efficiently and effectively, manage potential patient behaviors, and must be resourceful and have good attention to detail. All qualities are highly desirable, and may be preferred in the selection.

    High school diploma or GED required, Medical Assistant, Certified Nursing Assistant or other medical background, preferred. Previous work in a behavioral healthcare setting is also preferred. This position must embody the vision of the program, and must be helpful with staff and patients. A person may qualify for this position with an equal combination of education and experience, and will operate under the guidance of those with the proper degree and/or licensure/ certification.

    All personnel must pass a background check and not be convicted on any prohibited offenses per the Nevada Revised Statutes (NRS) 449.4329- NRS 449.4332. (Fingerprints must occur within 10 days after hiring.)

    • Murder, voluntary manslaughter or mayhem;
    • Assault with intent to kill or to commit sexual assault or mayhem;
    • Sexual assault, statutory sexual seduction, incest, lewdness or indecent exposure, or any other sexually related crime that is punished as a felony;
    • Prostitution, solicitation, lewdness or indecent exposure, or any other sexually related crime that is punished as a misdemeanor, if the conviction occurred within the immediately preceding 7 years;
    • Abuse or neglect of a child or contributory delinquency;
    • A violation of any federal or state law regulating the possession, distribution or use of any controlled substance or any dangerous drug as defined in chapter 454 of NRS, within the past 7 years;
    • A violation of any provision of NRS 200.5099 or 200.50995; abuse or neglect of an older or vulnerable person;
    • Any offense involving fraud, theft, embezzlement, burglary, robbery, fraudulent conversion or misappropriation of property, within the immediately preceding 7 years; or
    • Any other felony involving the use of a firearm or other deadly weapon, within the immediately preceding 7 years.

    New Admissions:

    • Required documentation for each new patient of the CTC:
    • Verification of benefits in coordination with the Billing Manager;
    • Signatures on all documents required for admission, however if intoxicated, just the Consent to Treat document must be signed with a plan to go back to the others when the patient is able to comprehend what they are signing;
    • Medical Assessment/Screen to rule out medical or psychiatric conditions that would be excluded from admission to the CTC, and/or would require additional assessment by the Nurse prior to accepting for admission; and
    • Documentation of the insurance verification, prior authorization and limits on length of stay and coverage, or other comments, will be completed in the patient's EHR, as well as informing the Case Managers and Nurses, as indicated.

    Communication:

    • It is anticipated that the Intake Coordinator will demonstrate professionalism by:
    • Responding to emails, as appropriate within 24 hours, however, if the email is urgent or an emergency, will respond within 1 hour;
    • Responding to phone calls within 24 hours; however, if the phone call is urgent or an emergency, respond within 1 hour;
    • Provide updates to third-party payers, (i.e. Anthem, HPN, Silver Summit, or Medicaid), in coordination with the Case Managers, on the daily status on the patients who are engaged in the CTC treatment modality, and plans to transition to the CDIOP, PHP, and all specialty doctor visits; and
    • Communicate with the team any insurance issues with the CTC.

    Required Documentation:

    • The Intake Coordinator must manage their time to complete all necessary documentation prior to leaving the facility.
    • Any incident reports must be completed within 4 hours of incident, at the CTC.
    • Ensuring that all insurance updates have been provided and there are no outstanding issues with those currently admitted to the CTC.
    • Complete and submit billing on all patients daily, as needed to assist the Billing Manager.
    • Complete the Intake Assessment (MA Assessment) and a note of the patient's status, medical and behavioral, following the admission.

    Discharges:

    • Communicate with the Billing Manager to ensure notification has been made to the insurance company, all discharges, placements and verify that all billing is completed.
    • Monitor bed availability to manage admissions scheduled and pending referrals from the Emergency Rooms.

    Required Meeting Attendance:

    • On-site CTC weekly meetings.
    • Staff Meetings
