Intake Services Representative - Home Care jobs in Ohio

Gracelove Home Health Services is seeking a dedicated and detail-oriented Intake Coordinator to join our dynamic team. As an integral part of our team, the Intake Coordinator will conduct interviews with referred clients and their families, exploring their needs regarding our home care services and identifying the specific services required. Additionally, the intake coordinator will conduct further assessments of referred clients in their homes and commence the creation of an initial activity home care plan using Gracelove Home Health Services software, ensuring seamless communication with assigned caregivers.

Please Note this position is flexible and will be scheduled as needed. Additionally, this is not a salaried position; however, a generous flat rate compensation will be provided after on-boarding each new client.

Responsibilities:

• Patient Evaluation and Assessment:
• Evaluate patients referred to home care services to determine their specific needs.
• Conduct comprehensive interviews with patients and their families to assess appropriateness for home care services.
• Documentation Management:
• Obtain and manage initial and ongoing referral documentation from physicians, insurers, and other healthcare providers.
• Ensure accurate and complete documentation to meet regulatory requirements and support the development of home care plans.
• Home Care Planning:
• Collaborate with healthcare professionals to develop patient home care plans that outline the frequency and duration of services required.
• Tailor care plans to address individual patient needs and preferences.
• Coordination with Service Providers:
• Initiate and coordinate care with service providers to ensure seamless delivery of home care services.
• Foster effective communication and collaboration among interdisciplinary teams involved in patient care.
• Health Record Management:
• Utilize health record management software to maintain accurate and up-to-date patient information.
• Ensure compliance with privacy and confidentiality regulations regarding health records.
• Qualifications:
• High school diploma or equivalent.
• Licensed Practical Nurse (LPN) credentials are required.RN credential is an edge.
• Strong organizational and communication skills.
• Ability to work collaboratively with healthcare professionals and service providers.
• Familiarity with health record management software.

Pay: $300.00 - $500.00 per day
