The LPN Intake Coordinator flexi position will cover weekend and holiday shifts. The weekends are typically 9-6 both Saturday and Sunday.
LPN Intake Coordinator evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients . Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues.
Job Requirements:
- LPN-Associate's Degree OR LPN-Diploma (Non-degree)
- Required: Basic Cardiac Life Support, Registered Nurse
- Preferred: Previous Home Care experience
