Luxe Home Health is a leading provider of Home Health services in the greater Chicago and Kansas City areas. Our brand-new location in Merrillville, IN is in need of filling the position of Home Health Visiting Physical Therapist. We are currently looking for Intake/Care coordinator to join our team.
The person we are looking for must have ability to read and understand medical records, verify patient's eligibility for Home Health Services and process multiple referrals. This position requires good computer and software skills, ability work independently, prioritize tasks and communicate closely with our office and field team members. Previous Home Health experience is required!
Responsibilities:
A. ADMISSIONS:
-Process incoming referrals, run INS plan, check patient's location and review clinical documentation.
- Coordinate decision about Acceptance of the patient for Home Health with Administrator or DON.
- Make sure all admission documentation is received and attached to the chart according to the Compliance regulations and requirements.
B. CARE and THERAPY COORDINATION: Includes following up hospitalized patients, setting up resumptions of care, getting medical records from facilities, following up with MD office to get home health orders signed, working closely with field clinicians and contractors to make sure all patients are assigned for the services, incoming calls, communication with the patients over the phone during whole period of services provided to support best quality of care, setting up patients with timely Therapy evaluations and visits (PT/OT/ST).
Required Skills and Qualifications:
1. Home Health Experience is required
2. Medical Background as LPN, Medical Assistant, RN, PT, PTA, Medical Coder - preferred.
3. Ability to read and understand medical records and evaluate patient's needs.
4. Ability to manage multiple tasks and still maintain friendly environment in the office
5. Detail-oriented and highly organized
6. Strong communication skills
7. Ability to solve problems, create professional relationships with colleagues and Referral sources.
