Collaborates and supports colleagues regarding research interests and co-curricular activities. Additionally, Instructor - Social Work typically reports to a department head. Has a Master's degree or is a PhD candidate in the applicable field. Has experience and is qualified to teach at undergraduate level and possesses the qualifications to participate in research. Provides case management activities, as well hands-on assistance to complete tasks; provides crisis intervention, and other supports as needed.Key Responsibilities
- Coordinates with Program Director and Supportive Services Social Workers in developing and carrying out service plan goals.
- Maintain and balance a caseload of 20-30 clients.
- Meet annual productivity expectation of 1074 hours or approximately 89.5 hours monthly.
- Complete home visits and telephone calls to assist with case management functions. Assist in accessing community resources, including making referrals and/or completion of applications or other required documentation.
- Accompanies clients to medical appointments; communicates with medical personnel around client needs as assigned.
- Assists clients in maintaining necessary benefits and health insurances include coordinating appointments, completion or obtaining required documentation and/or attending appointments.
Requirements- Associate degree in social work or a related field
- Social Work Assistant certification, required.
- One (1) year experience in case management
- Experience working with Seniors.
- Valid Ohio Driver’s license
