    RN Care Manager, Care Coordination, Discharge Planning and Utilization Review

    Southern Maine Health Care's Case Management department is looking for an energetic RN that is an excellent communicator with a solid sense of business conduct, who thrives on patient advocacy achieved through care coordination and planning.

    In this role, the selected candidate will use their leadership and critical thinking skills to partner with the interdisciplinary team for coordination and implementation of the optimal plan of care to meet patient goals.

    • A strong medical/surgical, intensive care or home health care background.
    • Previous case management and utilization review experience
    • Certified Case Manager Certification a plus
    • The ability to manage competing priorities necessary

    Position Summary

    Patient-focused coordination, negotiation, and procurement of necessary resources to address all identified areas of need for facilitation of achievement of quality, cost-effective outcomes. Acts as a patient advocate through collaboration with the patient and the integrative staff, internal and external to the organization. Through this collaboration, the individual's needs are assessed, goals formulated, and a plan of care implemented, monitored, evaluated and modified for positive outcomes. Actively participates in quality improvement and performance improvement processes related to the management of patient care and safety, and utilization of healthcare resources.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Bachelor Degree or currently matriculating preferred.
    2. License/Certifications: Current RN licensure from the Maine State Board of Nursing required.
    3. Experience: Five years of acute care clinical setting nursing experience preferred. Knowledge and experience in discharge planning, case management and utilization review preferred. Interest and experience in research through review and use of evidence-based practice preferred.
    4. Is responsible for maintaining a current knowledge of the nursing practices contemporary to the legal framework of the Nurses Practice Act, and in accordance with regulatory requirements.
