To serve as a leader, clinical resource, role model, coach, and facilitator for staff and department operations and function as a central resource to the Associate Vice President of Care Management to develop strategies which are efficient, effective, timely, patient-centered, and support appropriate financial reimbursement. Oversees daily operations of the Inpatient Care Coordination RN team, inpatient Social Work team, and oversight of program manager for UR, and clerical staff.
REQUIRED EDUCATION / EXPERIENCE:
RN with BSN and five years of inpatient experience in a hospital environment or Master’s degree in Social Work with 5 years of Care Management or healthcare experience. Current license or valid permit to practice professional nursing in Texas or must be licensed to practice Social Work in Texas (LMSW or LCSW). Five (5) years of acute hospital experience, two (2) year of supervisory experience that can include team lead or program management titles. CCM or ACM Certification required.
