Intake Coordinator
:
Overall Coordination of Agency Referrals and scheduling for Medicare and non-Medicare clients under the direct supervision of the Administrator. The Intake Coordinator is a resource person for referral sources and works closely with other members of the team to ensure adequate coordination and transition occurs to home health/hospice services. The Intake Coordinator should have extensive knowledge of the regulatory guidelines governing home health/hospice admissions.

The Intake Coordinator is responsible for managing all aspects of the patient/client intake process including:
1. Establishing and maintaining positive relationships with customers
2. Assures compliance of physician orders
3. Pre-authorizations and patient/client visits
4. Ensures appropriate and acceptable orders and diagnoses for home health/hospice referrals
5. Ensures all supporting documents such as physician Face to Face documentation, H&P, admission orders are received and uploaded to patient chart
6. Directs all daily patient/client referral and intake operations.
7. Enters patient information to Agency Documentation System and any approved communication tool for clinical staff and scheduling department of upcoming referral and scheduling within 20 minutes of notification
8. Maintains comprehensive working knowledge of community resources.
9. Gathers and tracks referral sources.
10. Assists in identifying opportunities for additional/improved services and methods.
11. Audits and monitors documentation for accuracy and completion.
12. Consistently represents BRiO HealthCare with integrity and professionalism to all internal and external customers.
13. Adheres to HIPAA regulations per Company policy and Federal regulations. Performs other duties and activities as delegated by the Administrator/Clinical Manager.
