RN Care Coordinator Position Summary

A RN Care Coordinator is responsible for the delivery of care to residents on the unit.  Their leadership role includes the following responsibilities: 

·      Manage the nursing care for all residents and delegate shift tasks to nurses.

·      Participate, observe, and validate the delivery of care to residents while identifying needs, participating in care plans, and administering care.

·      Maintain confidentiality of protected health information, including verbal, written and electronic communications.

·       Maintain confidentiality of protected health information, including verbal, written and electronic communications.

·       Perform other assigned duties as directed by the Director of Nursing or Administrator.

RN Care Coordinator Requirements

·      Current unrestricted RN license in practicing state

·      Current CPR/BLS Certification
