The Community Health Project Coordinator supports community health and health equity initiatives across the Hospital and/or Health System in order to further the Health System goal of improving health equity for all Hospital communities. The coordinator works under the direction of the Manager, Community Health Improvement and in collaboration with the system community health team to facilitate project management, programs and initiatives, surveys, data collection and reporting, evaluation, and resource development. The coordinator supports the Hospital and/or Health System and the collective impact partnerships including the Community Health Needs Assessments (CHNA) and Community Health Improvement Plans (CHIP) and participates in the system CHNA and CHIP process. The coordinator works in partnership with internal stakeholders and community partners who are collaborating on community health initiatives. The coordinator is an active member of the Office of Health Equity and Community Impact community health team.

EDUCATION
Bachelors degree with experience working in a health or social determinants of health related environment. Master's degree in public health, business, public administration, health policy, or healthcare management preferred.
EXPERIENCE
A minimum of 2 years of experience in community health, public health, or social work required. A minimum of 2 years of successful community engagement related work experience with community partnerships, community members, community-based organizations and/or governmental agencies preferred. Demonstrated ability to work in a team-oriented, highly iterative, and collaborative environment. Demonstrated ability to establish effective interpersonal relationships with a broad range of internal and external constituents. Experience with healthcare research preferred.
