What You’ll Do
Port Health’s Tailored Care Management (TCM) team provides whole-person, integrated health care through evaluation, assessment, care planning, coordinating, tracking, and monitoring outcomes of individuals served. The Care Manager focuses on closing gaps for physical and mental health, social determinants of health, and other non-health related needs. This position travels to Craven County and surrounding areas.
How You’ll Benefit
You’ll earn a annual salary of $51 ,000-$53,000k depending on skills and experience. This full-time position offers a flexible schedule Monday-Friday from 8 am-5 pm. We also offer benefits to those regularly working 30 hours or more per week.
