Health Informatics Manager manages a clinical biostatistics or health informatics team. Oversees the analysis, management and performance of health information data to aid patient care. Being a Health Informatics Manager monitors latest software and technology to keep processes up-to-date and efficient. Communicates with analysts, healthcare providers, and management to establish new processes and analyses, and to tailor current data processes to best help providers. Additionally, Health Informatics Manager requires a master's degree. Typically reports to a director or head of a unit/department. The Health Informatics Manager typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a Health Informatics Manager typically requires 3+ years of managerial experience. Population Health Case Manager - 50% remote (Shreveport)

  • Our team is seeking a Case Manager to work with us in Shreveport, LA.

    Case Managers are one of Mindoula’s key front-line roles in delivering our Population Health Program. Our Case Manager works closely with Mindoula’s members in the community to understand their behavioral and medical health conditions, as well as their social determinants of health needs, and help our members achieve better health outcomes.

    How you’ll contribute:

    • Conduct meaningful outreach, provide education, and drive member enrollment
    • In partnership with the member, create a Service Plan to guide engagement based on member diagnoses, results of screenings, as well as care gaps
    • Build measurable goals for members to develop skills and/or strategies for managing challenges and triggers to reduce hospital admissions/readmissions and use of the ER, and record outcomes
    • Coordinate community resources for each member to support them in achieving their goals as outlined in their Service Plans, created with an emphasis on meeting behavioral health and medical needs, and closing gaps identified in the social determinants of health
    • Act as an advocate and liaison between the care team (family, physician, facilities, and/or agencies) and member
    • Schedule appointments, build accountability, and follow-up services for members
    • Elevate critical incidents and information regarding any quality-of-care concerns
    • Report hospital diversions, productivity, and other outcomes weekly
    • Prepare and maintain all required records, reports, consents, and members’ diagnostic records

    Education & Experience:

    • Bachelor’s Degree required in nursing, social work, or other health and human services discipline
    • Completion of supervised fieldwork and experience in case management, health, or behavioral health preferred
    • Experience as a Certified Case Manager (CCM), Community Health Worker (CHW) or Peer Support Specialist (PSS), or Accredited Case Manager (ACM) preferred
    • Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, violence, and social issues is required

    Location:

    • This position is hybrid and requires up to 50% of time to be working in the field in the community
    • The employee is required to have reliable transportation
