Head of Home Care leads and directs the daily operations of a home health care agency. Establishes and implements quality standards for patient care. Being a Head of Home Care measures and monitors patient satisfaction and outcomes. Ensures that all operations and reporting requirements comply with regulations. Additionally, Head of Home Care requires a bachelor's degree. Typically reports to senior management. The Head of Home Care manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Head of Home Care typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Job Duties:
· Meet with clients one-on-one in their own homes
· Create and update clients’ plan of care based on their diagnosis and goals
· Perform semi-skilled nursing tasks-based client plan of care
· Educate client and family regarding specific practices to reach goals
· Coordinate client cares with physicians, case managers, and interdisciplinary team members
· Supervise Home Health Aide with caregiving instruction when necessary
· Daily documentation of client visits using electronic health records system
· Create and manage your weekly work schedule
· Participate in nurse on-call rotation
· 2-5 days per week (at your option); office in Plymouth
Qualifications:
· Active MN RN License in good standing
· Valid driver's license with good driving record
· Willingness to travel daily (local)
· Organization, communication, and time management skills
· Be able to utilize and navigate EMR system on a company-issued laptop
