Client is a young adult who lives near the mouth of Parley's Canyon. Cares include pain meds and TPN feedings.

Available Shifts: Monday - Sunday, 6 hours in the evening (flexible start and end time)

• Providing thoughtful care to client's based on personalized Plan of Care

• Providing in-home assessments for client's

• Monitoring and tracking changes in the client's conditions and needs, and keeping the Director of Nursing and any other appropriate personnel informed

• Proactively updating the client’s family on plan of care, health changes, and concerns

• TPN Feedings and additional tasks as needed
