Head Nurse - Quality Improvement - Home Care develops and implements various activities to improve overall quality of organization. Participates in and serves as a resource to quality improvement committees. Being a Head Nurse - Quality Improvement - Home Care tracks and analyzes trends and outcome reports and advises management on areas to improve upon. May be responsible to ensure all institute or government regulations are adhered to. Additionally, Head Nurse - Quality Improvement - Home Care requires a bachelor's degree. Typically reports to top management. The Head Nurse - Quality Improvement - Home Care supervises a group of primarily para-professional level staffs. May also be a level above a supervisor within high volume administrative/ production environments. Makes day-to-day decisions within or for a group/small department. Has some authority for personnel actions. Thorough knowledge of department processes. - Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
- Providing interventions with a focus on achieving realistic client outcomes.
- Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
- Administering medications as needed.
- Maintaining client records showing systematic assessments, planning interventions, and evaluations.
- Initiating referrals as needed to other health team members.
What we’re looking for:- Graduate of an accredited school of professional nursing.
- Current license as a Licensed Practical Nurse.
- Current CPR certification.
- Access to reliable transportation.
- Maintains confidentiality in all aspects of the job.
