• Black Hawk-Grundy Mental Health Center is a private, nonprofit, community mental health center serving Black Hawk and Grundy counties in Iowa.
• Community focused service for over 73 years.
• Established outpatient practice with 100 new patients monthly with case load of 6,000 active patients.
• Outreach services include medication monitoring, daily living skills training, crisis interventions or related services provided in the home or community.
• Multidisciplinary team of experienced Physicians, Psychiatry Nurse Practitioners, RN, Therapists, Community Support Specialists, with telehealth capacity
• Working directly with individuals and families to provide accessible Mental Health Services for short-term and long-term needs.
• Case Managers and Therapist complete majority of paperwork including court reports, routine letters, etc.
• Nursing staff provide triage for providers as well as complete prior authorizations for medication.
• A long-acting injection clinic 2 days per week to improve medication compliance, stability, and prevent re-admissions.
• Integrated home health for adults and pediatric patients
• Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination

• Rewarding work environment while building relationships with community mental health patient population
• Built in panel of adult, adolescent, and pediatric patients
• Average 16-20 patients per day
• Collaboration & mentorship provided by our multidisciplinary team
• Flexible scheduling options to accommodate work/life balance.
• Monday-Friday with evening hours available
• No in-patient responsibilities
• Community based-outpatient services, care coordination, evaluations, follow-ups, medication evaluation, and management services.
• Telephonic compensated after hours call coverage currently rotated monthly among ARNP’s
• Limited meetings required so you can focus on direct patient care.

• License and certification to practice as an Advanced Psychiatric Nurse Practitioner in Iowa
• Co-occurring Mental Health and Substance Abuse experience preferred
• Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
• Eligible to work in the United States
