    Description

    The Jamestown S'Klallam Tribe is seeking a full-time Case Manager Prevention Specialist to play a pivotal role in promoting well-being and preventing substance abuse in the youth of our community.

    Position Summary

    The Case Manager/Prevention Specialist is responsible for the chemical dependency and mental health needs to support and coordinate a broad range of Social and Community Services (SCS). This pivotal role combines case management and prevention efforts to address substance abuse among Tribal youth through the Icelandic model. This position designs and implements prevention programs, provides personalized support, and collaborates with various stakeholders to create a supportive environment for Tribal clients, children, and families. The position provides compassionate and professional social work to include crisis support to victims and their non-offending family members and/or caregivers.

     

    Essential Functions

    • Design and implement programs that provide constructive and engaging activities for youth, promoting positive alternatives and building a supportive environment to deter substance abuse.
    • Collaborate with community organizations to establish a cohesive approach to substance abuse prevention.
    • Organize recreational programs and events for youth and coordinating with volunteers.
    • Assist in planning and facilitating regular Recovery Groups and Retreats.
    • Perform clinical screening and strength-based assessments of children and families.
    • Provide coordination, referrals, and transportation to clients needing assessments, intake, or other social services.
    • Guide victims through criminal justice system and attend hearings as needed.
    • Assist with legal advocacy for primary victims and their families through the protection order process.
    • Work closely with Indian Child Welfare (ICW) team, attending team meetings and supporting families within the ICW system.
    • Attend regular clinical consultations with direct supervisor and case management team.
    • Develop and maintain service referral networks.
    • Assist in developing appropriate wraparound teams, plans, and implementing processes.
    • Manage and coordinate anti-substance use activities related to opiate, Juul or similar settlements or funding. Stay informed about legal developments and ensure that affected individuals receive appropriate support and resources.
    • Provide visits and/or transportation to clients and other community members who may have social and/or physical barriers.
    • Provide individual and group classes to increase education regarding gambling prevention, tobacco cessation, drug and alcohol prevention, positive self-care, cultural resources, cultural competency, nutrition, financial stability, and over all well-being.
    • Assist clients with applications for health benefits, financial assistance, other DSHS benefits, legal paperwork, licensing, past fines, SSI, Disability, etc.
    • Maintain database for grant reporting and tracking results.
    • Assist the SCS Director and Grant Manager with data management for grants.
    • Maintain confidential client files.
    • Attend family staffing meetings and create documentation in line with client action plans for SCS staff members.
    • Coordinate client care with the Culture department to incorporate traditional healing within treatment and action plans.
    • Perform additional duties as assigned.

     

    Requirements

    • BA and two years of Substance Abuse Disorder (SUD) experience (Additional SUD related experience and AA degree may be substituted in lieu of BA degree).
    • Experience in designing and leading substance abuse prevention programs.
    • Knowledge and understanding of Tribal governments and traditional customs and culture of Jamestown S’Klallam Tribe
    • Familiarity of legal court proceedings and family law, specifically as it relates to tribal members.
    • Familiarity with how the interplay of cultural issues (e.g., racism, gender) and poverty may influence how some people are affected by and respond to situations of domestic violence and/or sexual assault.
    • Ability to work independently as well as collaboratively.
    • Excellent organization skills and the ability to prioritize multiple tasks.
    • Basic computer and software knowledge including but not limited to Microsoft Office.
    • Strong interpersonal skills including effective verbal and written communication.
    • Ability to travel domestically or internationally.
    • Valid Driver’s License
    • Must be able to successfully pass criminal background investigation.

     

    Preferred Qualifications

    • Experience with working with individuals and families with diverse backgrounds as well as barriers to self-sufficiency including but not limited to, substance abuse and dependence, mental health needs, homelessness, unemployment.
    • American Indian/Alaska Native preferences apply.
    • Knowledge of evidence-based prevention strategies and familiarity with the Icelandic Prevention Model is a plus.
