Care Coordinator works with the primary care provider and all members of the health care team to manage patient care in the clinic setting. Uses guided workflow to conduct annual wellness visits to reduce the severity of chronic disease and avoidable acute illness. Provides chronic disease education and self-management coaching to assigned patients. Navigates patient transitions of care to post-acute setting. Current licensure as a Registered Nurse required. Previous experience caring for chronic disease patients required. 3-5 years experience in clinical or community health settings, Care Coordination, Case Management or Home Health experience preferred.
