The Field Service Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Field Service Coordinator typically requires 3 years experience in the related area as an individual contributor.     • SUMMARY: The Service Coordinator Aide is responsible for providing administrative and clerical support and internal monitoring tasks for the Service Coordination Department. This position is responsible to track requests of intakes, transfers, referrals for services, and tasks completed by Service Coordination Department staff to ensure deadlines are met in accordance with State regulatory guidelines. This position is responsible for completing documentation for scheduled monitoring of service coordinator services, documentation, and tasks to ensure program compliance and quality of work. The Service Coordinator Aide will also process paperwork and files; scan documents; handle general office responsibilities; and prepare documentation for all meetings and routine support to the Service Coordination Department. This position will assist Service Coordinators and the Community Relations Director as needed, especially with any unbillable processes. This position has a limited case load of children and adults in services for whom they are the primary facilitator of person-centered services; coordination of service delivery; identifying resources in response to clients' needs; and performing service coordinator duties (Targeted Service Coordination) for assigned children in a person-centered environment. The Service Coordinator Aide is responsible for serving as a receptionist backup, when needed.
    • ESSENTIAL DUTIES and MINIMUM QUALIFICATIONS:

    1. Complete necessary paperwork assigned by the Community Relations Director which may include but is not limited to the following: typing memos, meeting minutes, meeting notices, routine filing, creating various reports, mailing and emailing correspondence, etc.
    2. Maintain various staff, client and agency databases and electronic files.
    3. Track State regulatory and agency contract deliverables including, but not limited to, track requests for transfers, referrals for services elsewhere, and tasks completed by the Service Coordination Department staff to ensure deadlines are met in accordance to State regulatory guidelines.
    4. Track and complete documentation for scheduled monitoring of Service Coordinator services, documentation, and tasks to ensure program compliance and quality of work within established timelines.
    5. Perform billing functions for children in service in accordance with established agency, state and federal rules and regulations and within established timelines.
    6. Perform the following Service Coordinator duties under the supervision of the Community Relations Director: provide direct programmatic assistance in program; develop preliminary and annual Individual Plans; perform monitoring of client services and supports as designated by regulations; act as a knowledgeable liaison between clients, families, guardians, program staff, and other agencies.
    7. Pass the Medication Administration test with no more than two attempts and administer medication to clients according to physician orders and using appropriate administration techniques as outlined in the Medication Administration course, if needed.
    8. Must be able to perform Lifting and Transferring techniques as trained and pass performance testing. The employee may occasionally lift and/or push and pull up to 50 lbs. Lifting may require floor to waist, waist to shoulder, or shoulder to overhead movement.
    9. Driving of agency vehicles as necessary for transportation of clients and agency business as assigned.
    10. Must, with or without reasonable accommodation:
    • Be able to regularly sit, walk, stand, climb up and down stairs, physically restrain clients pursuant to training (when necessary) and sit at a computer for long periods of time and repeat the same hand, arm, or finger motion many times (typing, data entry).
    • Communicate through speech with individuals in service and public in both quiet and noisy environments.
    • Operate a personal computer and related software.
    • Tolerate various levels of stress and meet deadlines with severe time constraints.
    • Demonstrate specific vision abilities which include close, distance, and depth perception and are able to read handwritten or typed material, and able to adjust focus.
    • Work and concentrate in both noisy and quiet environments.
    • Squat, stoop, kneel, and reach above the head. Forward motion will occasionally be required.
    • Work inside the building a majority of the time.
    • Essential Duties include the Physical Demands listed below.

    ADDITIONAL DUTIES AND RESPONSIBILITIES:

    1. Coordinate initial and all other Individual Family Service Plan / Individual Plan meetings to develop a plan based upon client needs, which includes the implementation and monitoring of services and supports. Ensure schedules of service changes are completed, submitted as necessary, and expenditures are within guidelines established by the State of Colorado.
    2. Obtain program accreditation through the Colorado Department of Early Childhood and Colorado and other regulatory agencies, and maintain program approval with few, if any, deficiencies for all assigned areas of responsibility.
    3. Ensures appropriate maintenance and completeness of individual's master files including the periodic purging and/or destruction of records in accordance with agency policies and procedures. This includes reorganizing and performing upkeep of file room periodically.
    4. Responsible for prior-authorization of services within established timelines.
    5. Coordinate various department and agency meetings, i.e. Bent, Crowley and Otero County Early Childhood Council, Child Development Services, and Early Head Start, and Santa Fe Trail BOCES.
    6. Maintain regular contact with all persons served to assess satisfaction with services, and advocate in collaboration with and/or on behalf of the client to ensure the consumer's rights to access appropriate services and opportunities. The Service Coordinator will act as a knowledgeable liaison between individuals receiving services, families, guardians, program staff, other agencies, providers, and the agency.
    7. Maintain all programs' databases, inputting vital information, and compiling reports as required by agency, state, and federal rules and regulations.
    8. Participate in scheduled Service Coordinator meetings and training sessions to maintain awareness of current issues which may impact the coordination of cases for individuals in service.
    9. Maintain strict confidentiality of information contained regarding clients and staff.
    10. Answer telephones and operate and maintain various office equipment and computer systems.
    11. Establish a good line of communication with all persons involved with the individuals in services, including but not limited to staff, parents/guardians/providers, case managers, and health services, etc.
    12. Due to the nature of this position, regular attendance and promptness to assigned shifts is essential.
    13. Participate in staff training activities and staff meetings, as outlined in the agency's staff development curriculum.
    14. Plan and participate in agency functions and events.
    15. Work flexible schedules or within various environments as agency and supervisor deems necessary. The responsibilities of this position may require staff to be able to work irregular hours, extended hours, and/or be available for nights, weekends, holidays, etc.
    16. Safeguard all agency funds and property.
    17. Perform job responsibilities and tasks with very strong clerical skills, attention to detail, organization, time management, and quality technical writing skills.
    18. Must be able to attend meetings in the evening and at other locations or at irregular hours.
    19. Follow agency procedures when purchasing items on behalf of the agency.
    20. Follow agency policies and procedures in carrying out duties.
    21. Maintain confidentiality per federal, state, and agency policies and procedures.
    22. Advise supervisor of concerns and problems that arise within the agency.
    23. Performs other duties as assigned by the Community Relations Director, CEO, and/or designee.
    24. Other reasonable duties as may be assigned.

    QUALIFICATION REQUIREMENTS: The requirements listed below are representative of the knowledge, skill, and/or ability necessary for success. An individual must be able to perform each essential duty satisfactorily.

    MINIMUM/PREFERRED QUALIFICATIONS:

    Minimum: This position requires a High School diploma or GED AND two years' experience in a related field. Must be at least 18 years of age. Persons hired into this position must be licensed drivers and have reliable transportation. If the current license is from out-of-state, a valid Colorado Driver's License is required within ten days of hire. Background check, drug and alcohol check, reference check, and motor vehicle checks must be passed. Current motor vehicle insurance is required. Driving is an essential job duty.

    Preferred: Associate's Degree and three years of experience in a related field such as Psychology, Social Work, Education, Early Childhood Education, Medical, Community Service, Human Services, Criminal Justice, Rehabilitation, office management, etc. Working knowledge of Windows and MS Office software and Google Workspace.

    The Service Coordinator Director provides supervision of this position. In general, this position requires that the individual work independently approximately 85% of the time.

Able to develop effective working relationships with staff, individuals in service, and the community. Good communication skills and being able to work with others as a team is a must.
